Any student who wants to play a sport for HMS, please see required paperwork and information below.

Athlete Checklist

• Complete all required forms

  • Physical Form

  • Release & Student Drug Testing Consent Forms

• Copy of Insurance Card

• Copy of student-athlete's birth certificate (6th grade ONLY)

Important Notes

• ALL PAPERWORK MUST BE COMPLETED AND TURNED IN TO BE ELIGIBLE TO FOR ANY TRYOUT OR PRACTICE

• Student-athlete and Parent must sign in ALL appropriate areas on required forms.

• Forms must be turned into the Athletic Director or Coach BEFORE tryouts begin.

• Physicals required for sports participation for the next school year MUST be dated on or after MAY 1st.

• If an athlete turns in forms to one sport and tries out for another, the athlete DOES NOT have to complete new forms.
